Low Cost Medical Care: JPS Discount Program

JPS Discount Program

JPS is offering a JPS Discount program for those who have income in the 251 to 300 percent of federal poverty guideline range.  Participants in the JPS Discount program pay a fixed fee for clinic visits and receive an 80 percent discount on all covered services.  Eligibility is based on household size and gross monthly income according to program guidelines. The application process and required documentation are the same as for the JPS Connection.

Patients are expected to pay clinic fees at the time of service.  These fees are at a greatly reduced price and include the physicians’ fee.  Examples include:

            $25 co-pay for a primary care visit

            $35 co-pay for a specialty clinic visit

            $35 co-pay for an urgent care visit           

 

These co-pays include routine radiology performed at the clinic sites and routine laboratory (e.g. chemistry and hematology testing) when performed on the same day as the clinic visit. 


For all other services, including emergency room visits, outpatient surgery, inpatient hospitalization, advanced laboratory testing and diagnostics and pharmacy, participants in the JPS Discount program will receive an 80% discount.

Eligibility Requirements:

A review of the patient's required documentation is performed on any patient entering or returning to JPS Health Network who has no verifiable third party coverage. The purpose of the interview is to determine if the patient is potentially eligible for a state or federal program, or for the JPS medical program, JPS Connection, to assist them with their medical costs.

To determine eligibility the patient is asked to provide certain documentation. Review the Documentation Requirements (Documentation Requirements in Spanish) for a list of the specific types of documentation required for eligibility.

Medically Indigent Program

Patients who are uninsured or underinsured with gross income above the Federal Poverty Guidelines who have outstanding medical bills and meet the program guidelines can be financially screened for this program. Please call for more information.

How can the patient apply?

A financial screening appointment can be scheduled by calling 817-222-2577. Walk-ins will be worked in if time allows. The patient may also apply through the mail or online. Upon review of the online application additional JPS Health Network eligibility forms will be forwarded via mail or email. Mailed, faxed or email scanned copies of the required documentation along with the application will be accepted.
